Quality Care Manager (RN) - Hospice

Are you looking for a Remote opportunity in Quality Care Management? 

Our Hospice team is growing and looking for an Registered Nurse to add to the team. Combing your skills in both Hospice bedside care and management.

The Quality Care Manager is responsible for reviewing patient assessments to ensure appropriate 
and effective care is provided to each patient in compliance with agency policies, procedures, and 
regulatory requirements. Trends or patterns of inaccuracy identified by the QCM will be 
communicated to the Quality Director to assist in the development of the performance improvement 
and education plan.
 
Responsible for processing and reviewing clinical documentation- RN license in CA, and Compact license needed

Essential Job Functions/Responsibilities:

    • Reviews electronically submitted documentation and provide feedback to clinicians regarding deficiencies or Hospice Item Set (HIS) corrections to ensure the medical record reflects compliance with hospice eligibility guidelines.
    • Ensures visit utilization is supported by individual patient assessment
    • Reviews additional orders for appropriateness and compliance
    • Ensures clinical documentation accurately reflects recertification/discharge decisions. Prepares hospice CTI/recertification workflow prior to IDG meetings.
    • Reviews and approves Initial Hospice Plan of Care within HCHB
    • Reviews HIS for inconsistencies and locks timely
    • Collaborates with clinicians as necessary regarding documentation
    • Partners with the Quality Director on a variety of quality initiatives and attends quarterly QAPI meetings 
    • Monitors performance improvement initiatives regarding patient outcomes
    • Maintains working knowledge of hospice regulations, conditions of participation and HIS

Position Qualifications:

    • Must be a graduate of an accredited School of Nursing
    • Current unencumbered license to practice as a Registered Nurse
    • Minimum of 1 year of hospice experience with a working knowledge of Title XX11, Medicare Hospice Conditions of Participation, and HIS
    • Excellent written and oral communication skills required
    • Ability to handle stressful situations/deadlines.
    • Strong computer and software skills with knowledge of Home Care Home Base a plus.
$85,000 - $90,000 a year
