Over the last year, we've built the leading virtual cardiac recovery and management platform for members with cardiac conditions. For every member we serve, we add an average of five years to their lifespan.

About You

As heart disease remains the leading cause of death, you are deeply committed to ensuring that patients receive the care, guidance, and support they need to navigate their health journey with confidence. You believe in breaking down barriers to care, advocating for patients' needs, and making high-quality cardiovascular care more accessible — especially from the comfort of home.

You’re known for your empathy, strong communication skills, and ability to build trust with patients and caregivers. Your problem-solving mindset and ability to collaborate with clinical teams fuel your success in helping patients overcome obstacles, access critical resources, and actively participate in their own care.

The Community Health Worker will serve as a frontline advocate, connecting patients with chronic conditions to essential healthcare and community resources. This role involves conducting person-centered assessments, facilitating access to services, and promoting patient self-advocacy to enhance health outcomes.

 

Responsibilities

  • Conduct comprehensive assessments to understand patients' strengths, needs, goals, and preferences, including cultural and linguistic factors.
  • Facilitate patient-driven goal setting and develop actionable care plans in collaboration with the healthcare team.
  • Coordinate with healthcare providers, community-based organizations, and caregivers to ensure seamless delivery of services.
  • Assist patients in navigating the healthcare system, including scheduling appointments and understanding treatment plans.
  • Provide culturally appropriate health education to promote disease prevention and management.
  • Identify and address social determinants of health by connecting patients to community resources such as housing, transportation, and food assistance.
  • Empower patients to develop self-advocacy skills and actively participate in their healthcare decisions.

Your Past Experience

  • High school diploma or equivalent; associate degree in health or human services preferred.
  • Certification as a Community Health Worker or relevant training as per state regulations.
  • Experience working with diverse populations, particularly in underserved communities.
  • Strong interpersonal and communication skills.
  • Knowledge of local community resources and healthcare systems.
  • Ability to work independently and as part of a multidisciplinary team.
