Remo Health is hiring a full-time, 100% remote, Geriatric APC (Nurse Practitioner or Physician Assistant  ). This is a key role that will help us continue on our mission to transform the standard of dementia care.

About the Role

As a Geriatric Advanced Practice Clinician, you will report directly to our Medical Director and play an integral role in our ability to pursue our mission with the clinical team. You will be part of an interdisciplinary team seeing dyads for both new and established visits. You will be managing dyads within the framework of the 5M’s (medications, mentation, what matters, mobility, multi-complexity) with a focus on their cognitive disorder. What You’ll Be Doing

  • Evaluating and helping manage a panel of patients with cognitive disorders and their caregivers, with the full support of an interdisciplinary team (Registered Nurse, Pharmacist, Social Worker, Care Navigator).

  • Identifying areas of improvements in the care model and technical platform so we can continue to improve quality of care, dyad satisfaction, and the provider experience.

  • Collaborating with the clinical team to deliver on our key clinical KPIs — e.g. quality targets, utilization metrics, patient/provider satisfaction, etc.

  • Augmenting the care delivered by the medical director by assisting with panel management tasks along with patient facing time.

  • Collaborating with the Medical Director to develop, implement, and maintain a curriculum of regular educational conferences for clinical providers (e.g. clinical case conferences and guest lecturers).

You May Be a Good Fit If You

  • Have experience caring for older adults with cognitive disorders and/or younger patients with early-onset cognitive disorders in a neurology setting.

  • Have experience in dementia and related cognitive disorders.

  • Graduated or completed from one of the following:

    • Accredited US Nursing School 

    • Accredited NP Program 

    • US PA program  

    • US National PA Certification (PA-C).

  • Have experience in geriatrics and/or palliative care. Have experience in advance care planning and serious illness communication with patients and care partners.

  • Have experience with and thrive working in interdisciplinary care teams.

  • Enjoy working in a telehealth environment.

  • Believe in a holistic approach to care for patients with dementia and their caregivers.

  • Are passionate about educating families and other clinical providers.

  • Think systematically and enjoy breaking down complicated problems into manageable initiatives.

  • Have strong problem-solving skills and the ability to navigate challenges with creativity and flexibility.

  • Have excellent communication and interpersonal skills, with the ability to build strong relationships.

  • A License in OH, WA, NV, FL (Preferred)

  • A proven track record of working effectively with interdisciplinary teams

  • A passion for building and iterating on a novel care model and technical platform.

  • Strong leadership and team work skills, and leading by example

  • A belief  that the current state of dementia care is unacceptable and want to build a better care model for all the families who need it.

  • This job is 100% remote, providing schedule flexibility.
